IUP Sports Medicine Department
Returning
Student-Athletes
All of the following information is
required to be on file in the Sports Medicine Department before you will
be allowed to participate in intercollegiate athletics at IUP. If any
information is missing or incomplete, you will not be allowed to
practice, and you will not be issued equipment or a locker.
Pre-Participation Physical
(The
pre-participation physical is not required for returning athletes. If
you would like to have one, please have your physician complete the
physical and return it with the rest of your medical information.)

If you have questions, please
contact the certified athletic trainer for your sport. All information
can be sent directly to your ATC when it is complete. All information
for the school year is due by July 1, 2007.
