Please complete this form when requesting anything from our office and fill out all sections dealing with your particular request. Please allow 2-3 business days for completion of your request and pick up your document at the OIA Front Desk. If you want your document to be mailed -specify below.

These documents include: Current I-20 or DS-2019 Acceptance letter or letter from department advisor indicating Change of level or Change of Major.
