    Current Students

    • Check-In Form - In order to comply with USCIS regulations concerning immigration, each student must complete a check-in form each semester and/if each time you move. Please complete and return this form to Wallce Hall Anex within 10 days of arriving and/or moving and each time you move. Students receiving partial tuition waivers must have a current form on file to be eligible to receive the waiver. Download here - (PDF format, 66 kb)

    • F-1 Curricular Practical Training - This is for students who will be completing a paid internship or practicum. To be approved, you must have an internship approved by your department, register for credits, and have a letter from a sponsoring company.    Download Forms here - (PDF format, 170 kb) or Web-Presentation here.
       
    • Health Insurance Waiver - This form is due by the end of ADD/DROP period each semester. Download here - (DOC format)
    • MA Thesis or PhD Comp verification Form - If your are not registered for dessertation credits but are completing an MA Thesis or preparing for comprehensive exams please complete this form. Download here - (PDF format, 64 kb)
    • Program Extension - If your I-20 is going to expire, please complete these forms at least 2 weeks before your end date. Download here -(PDF format, 167 kb)
    • F-1 Optional Practical Training - An F-1 student who has maintained lawful F-1 status in a post-secondary level for at least 1 academic year may be eligible to apply for OPT work permission. For more information, please click here or Web-Presentation here. .
    • J-1 Academic Training - Academic training is work, training, or experience related to a student’s field of study and offered through the OIA International Student Advisor.  You must have permission and documentation prior to starting a paid position.     • Readmission Form - If you are a readmission student, please complete this form and return it back to OIA for appropriate immigration documents. Download here - (PDF format, 52 kb)
    • Reduced Course Load - Students in their last semester of coursework, those having difficulty in the middle of their first semester, and those with medical excuses can apply for a reduced course load. Wait for approval to drop any classes, otherwise you will be out of status.  Download here - (PDF format, 67 kb)
       
    • SEVIS Transfer form - For all IUP international students who have graduated, will begin another degree or continue their degree at another school can refer to these instructions on how to transfer their SEVIS record. Download here - (PDF format, 101 kb)
       
    • Replace lost or stolen I-94 Card - Download here - (PDF format, 57 kb)
       
    • Social Security Work Authorization Form - All new incoming international students can only be issued a social security number when they have been hired for a job. Bring this form to your employment supervisor for a signature and then to the OIA for a signature before visiting the Social Security office. Download here - (PDF format, 93 kb)

    Prospective Students

    • Affidavit of Financial Support - It is necessary that IUP have adequate information regarding an international student's financial resources. The purpose of this Affidavit of Financial Support is to demonstrate that you, or your sponsor, are capable of full financial responsibility for your tuition, fees, books, housing, etc., during your undergraduate studies at IUP. This information will be kept confidential. Please convert all sums to U.S. dollars.

    • International Transfer Form - Students currently studying in the United States or on Optional Practical Training must have their current Foreign Student Advisor complete the form and return it to IUP. I-20s will be issued after receiving the transfer form and SEVIS transfer out date. Download here - (PDF format, 76 kb)

    • Immunization Certificate - The IUP Health Center asks all IUP students to complete the form and turn it in to them upon your arrive in the states. It is not mandatory to fill out. Download here - (PDF format, 146 kb)

    • Spring 2008 Transportation from Airport - All new students can reserve a spot on shuttle buses from the Pittsburgh International Airport to Indiana. It costs US$40 per person and reservations must be received via fax or email by January 2. Download here - (PDF format, 55 kb)
