Basic Group Life Insurance

 ELIGIBILITY - Permanent employees become eligible after a 90-day waiting period. They also become eligible for the work-related accidental death coverage unless the surviving spouse, minor children or parents are entitled to benefits under Act 101 of 1976, which provides death benefits to survivors of law enforcement officers, firefighters, ambulance, and rescue squad members killed in the performance of their duties.  Temporary employees are not eligible.

Voluntary Group Life and Personal Accident Insurance

Available to all full-time permanent employees including temporary faculty employed for a one-academic year contract, and permanent part-time employees including temporary faculty employed for a one-academic year contract who works 50% time.

  • 100% employee paid
  • Employee term life and personal accident insurance available in $10,000 increments; maximum coverage $500,000
  • Spouse term life and personal accident insurance available in $10,000 increments; maximum coverage $100,000
  • Children term life and personal accident insurance available in amounts of $5,000 or $10,000 (NOTE:  Dependent child is defined as an unmarried child from live birth through age 20, age 24 if a registered student in full time attendance at an accredited educational institution and dependent on you for support and maintenance.)

Voluntary Long-Term Disability Insurance

Available to all full-time permanent employees including temporary faculty employed for a one-academic year contract, and permanent part-time employees including temporary faculty employed for a one-academic year contract who works 50% time.

  • 100% employee paid
  • Income protection equal up to 60% of gross annual base salary
  • Amount offset by retirement benefits, workers' compensation, social security, and paid leave with a guarantee of 10% of long-term disability benefit or the amount of $100/month, whichever is greater
  • Option of 90-day or 180-day elimination period (effective January 1, 2006)
  • $.265 per $100 of monthly covered earnings for 180-day elimination period;  $.333 per $100 of monthly covered earnings for 90-day elimination period
