Essential Information for International Students:

International Students in J or F Visa Status: Waiver deadline is end of drop/add each semester

The deadline to return the insurance waiver form and related documents is the end of the "add drop" period each fall and spring (or the first day of the second week of classes, whichever is later).  Students who do not return a waiver form with documentation of their insurance coverage will automatically be billed the cost of Consolidated Health Plans, Inc. and the cost of IUP's Health Fee A.  (See fee page for health center charges)  Waiver forms and documents must be provided each fall and spring.  Please contact the Office of International Affairs if you have questions.

The campus policy regarding insurance for international students:

Indiana University of Pennsylvania requires J and F visa holders to have health insurance.  Each semester of enrollment students must provide proof of insurance coverage of at least $50,000 major medical coverage, $10,000 for medical evacuation and $7,500 for repatriation of remains.  The deductible may not exceed $500.00 per accident or illness.  As defined by federal regulations, students who possess J visas must also carry the same level of insurance coverage for a spouse and/or dependent child(ren).  Students may select an insurance policy that is in compliance with the stated requirements.  Students who are F and J visa holders will be billed for the Consolidated Health Plan, Inc. student group health insurance policy and IUP's Health Fee A at the beginning of each academic term. (See fee page for health center charges) Students that provide proof of insurance within the specified deadline period (last day for add/drop in fall, spring and summer) will be waived from these fees and will have the charges removed from their student account.  The student must maintain valid medical insurance for the entire period of time of enrollment at Indiana University of Pennsylvania.  If at any time the university becomes aware that the student is not covered by an insurance plan that is in compliance with the policy, the student will automatically be billed for the student group health insurance plan at the earliest possible enrollment period.  [Insurance plans must be purchased from a company that has a U.S. telephone number and the company must be licensed to sell insurance in the United States.]

Students who have Consolidated Health Plans, Inc. must also have Pechan Health Center's "Fee A" in order to receive clinical care.  (See fee page for health center charges)

If you cannot open and print a Word document contact Pechan Health Center at health-inquiry@iup.edu and the form will be sent to you.  The form is also available at Pechan Health Center or the Office of International Affairs.  Complete and return the form if you currently have insurance coverage that meets the minimum standards.  If you do not complete and return this form by the deadline for each semester you will automatically be enrolled in the Consolidated Health Plans, Inc. insurance plan.

Consolidated Health Plans, Inc. is a quality and cost effective student  insurance plan offered to IUP students.  Consolidated offers insurance as well as a discount prescription card (Express Scripts) that is accepted at the overwhelming majority of pharmacies in Indiana.  For more information contact Pechan Health Center at (724) 357-6475 or email us at health-inquiry@iup.edu. This insurance plan meets the requirements for J and F visa holders.  The plan is available to all graduate students enrolled in one or more credits AND any undergraduate enrolled in 6 or more credits. 

INSURANCE PLAN INFORMATION:

Consolidated Health Plans, Inc. (CHP) is the endorsed vendor for student group health insurance at IUP.   We highly recommend that all students have health insurance.  This plan meets the requirements for J and F visa holders.
All graduate students, part-time and full-time, are eligible for the plan.  Undergraduates who are enrolled in 6 or more units of credit are eligible for the plan.  Brochures are available at Pechan Health Center. The Consolidated plan does require that students and spouses seek medical care from Pechan Health Center for most illnesses and injuries (see the brochure for more information and exceptions to the rule). In order to seek clinical care at Pechan Health Center students and spouses will be required to pay Fee A.  Children covered by the Consolidated plan are not seen at Pechan Health Center and therefore not required to pay Fee A.
