Fall 2007 and Spring 2008
 

                    Health Fee A - $125 per semester

    All full-time undergraduate students pay this mandatory fee.  This fee is only able to be waived in certain circumstances (click here for more info about waiving the fee).   This fee covers the following:

    • Most of the services provided by physicians, nurse practitioners, and registered nurses, including procedures and supplies, are included with Fee A.
    • All programs and services listed in Fee B are included
    • Most health center visits involving a physician, nurse practitioner or registered nurse will incur a $10 co-pay per visit
    • Self care cold and wound clinic supplies and medicines (no charge)
    • TB testing is free for Fee A if using mass clinics.  $15 fee if not during clinics.
    • Pregnancy testing alone ($10 plus $10 co-pay)
    • Emergency Contraception only (Effective October 2006: Now available without a provider visit.  $25 medicine charge with no co-pay.  Sign in and request from front desk.)
    • Minor laboratory tests performed at Pechan Health Center (no add'l charges)
    • Woman's yearly gynecological exams (laboratory tests are not included in health fee and will require insurance or payment of lab costs) ($10 co-pay)
    • Ambulance transportation to Indiana Regional Medical Center at no direct cost to you or your family
    • Eligibility for allergy clinic injections (serum provided by student) are available for Fee A payers at a cost of $35.00 per semester.
    • Eligibility for the student health insurance plan.  Must be an enrolled graduate student (1 or more credits) or an enrolled undergraduate (6 or more credits). 
    • Fee A will be charged to all enrollees of university required student health insurance.

    Health Fee B - $21 per semester

    This partial fee is mandatory for all part-time undergraduates and all full time graduate students.  The fee is able to be waived in certain circumstances (click here for more information about waiving the fee).  This fee allows:

    • Use of the Self-Care Cold & Wound Center (no charge)
    • Flu Shots (no charge)
    • Van transportation to health care appointments within three (3) miles of Indiana campus (no charge)
    • TB Testing $15.00
    • Pregnancy testing $30.00
    • Emergency Contraception only (Effective October 2006: Now available without a provider visit.)  $35 medicine charge with no co-pay.  Sign in and request from front desk.)
    • Physicals $30.00  Call to inquire if the physical you need is offered at the health center. (No additional co-pay)
    • Access to programs and services provided by Health AWAREness

    Note to Fee B payers: You are eligible for the student health insurance plan when you upgrade to Fee A.  Must be an enrolled graduate student (1 or more credits) or an enrolled undergraduate (6 or more credits).  If you enroll in the plan you will need to upgrade to Fee A. 

    Fees for special services

  • Meningococcal meningitis vaccination $90.00.  No appointment needed.
  • Physicals $30.00. Teachers Certification, Athletic, Child Development, Drivers License, SCUBA.  Check to see if specific physicals are provided.  No appointments are used, physicals are done as walk-in.
  • STD Testing lab fee $15.00. plus $10 co-pay.  Covers visit and laboratory work. ($45.00 if the student has not paid Fee A). 
  • Medications (Varied).  Medications require a co-payment by the student that vary depending upon the product. Students may also request a prescription that can be filled at the pharmacy of their choice.  The health center does not operate a pharmacy.  We carry a select inventory of medications ("formulary").  Prescriptions from medical providers outside of Pechan Health Center cannot be filled by our office.
  • Records Fee $15.00.  Student interns can have their records reviewed and stored at the health center for internships that require medical data.
  • Visit Fee $30.00. Students who have not paid Fee A may pay "per visit" if they enroll in Fee B and choose to be seen by a provider.  Medications are not included.  There is no co-pay with this fee.

A portion of the health fee (both Fee A and Fee B) support the Center for Student Life's Health AWAREness Department (Health Awareness). The Health AWAREness staff is located in 106 Pratt Hall.  All IUP students are eligible to participate in the programs and services offered by the Center for Student Life.
